Lutheran Social Services of WI and Upper MI is currently seeking a full-time, benefit eligible Human Services Professional to join our Family Partnership Initiative program.
Family Partnerships Initiative Program (FPI) is a program created to serve at-risk children and their families, as well as children who have substantial limitations in their daily activities, who need support to remain in their home and community. The provider supports by working as part of a team in the capacity of providing Family/Unpaid Caregiver Supports and Services through the CLTS Waiver program.
FPI provides education, training, and support in the home and/or community to an Unpaid Caregiver designed to increase confidence, stamina, and empowerment needed to support and care for their child. The provider supports Families and/or other Unpaid Caregivers to meet Long Term Goals families have established for the CLTS Outcomes. Support may be provided in the areas of:
Under direction, perform work involving the delivery of professional human services to individuals, families, and/or groups. This work may be completed in a variety of settings (e.g. home, school, office, community) utilizing various forms of service delivery (e.g. in-person, audio, visual) per the service plan. Performs other duties as required.

EDUCATION AND/OR EXPERIENCE:
Bachelor's degree in relevant area of human services.

CERTIFICATES, LICENSES, REGISTRATIONS:
The incumbent of this position must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
